I have a tough time with that as well and it all depends on what style of play I am using for my Hawke. You could tell that despite everything Varric still has some feelings for his brother. Unless I am playing a nasty version of Hawke I usually let him live figuring that the idol is what in the end caused him to go from just being a greedy dwarf to being an insane one.

It's his brother, and his choice IMO, so it all comes down to whether or not Anders is there and called on. Doesn't really matter to me either way. I'm over the betrayal thing enough to accept Varric sparing him, but do not care enough to stop Varric from killing him, either.

All that matters is that he won't be a danger to others. He's gone mad, but he's not a mage; take away the knives and you're fine. This was one of the easiest decisions in the game for me. If anything Hawke should be the one to do it if Varric can't. It's only a hard decision for Varric if you bring Anders and ask his input. For me the hardest decision in the game was whether to kill Anders or not when he was a friend. Or maybe if I should kill off Carver or make him a warden. I have him killed without hesitation.

While I wouldn't have the hangup on the personal sting of betrayal that would no doubt affect Varric, I don't value Bartrand's life enough to try to encourage Varric to spare him either.
